- 1 Team of 4 players: includes lunch, 2 Carts, 8 Mulligans (2 per player), 4 tournament apparel, photo for each player
Black Team: $400
- 1 Team of 4 players: includes lunch, 2 Carts, and 4 Mulligans (1 per player)
Individual Golfer: $100
- 1 Player: includes lunch

How to register:
Step 1
Pay online through our secure connection by clicking the "Donate" button below.. You can use PayPal, credit card, or bank draft. Or pay by mail.
You may wish to read the instructions on our Donation Page before you click on the Donate button if you are paying by credit card.
Step 2
Submit your information to our organizers by e-mail at maizefoundation@gmail.com. Please include each player's:
- Name
- Shirt size
- Mailing address
